To send a specimen, please print and complete the appropriate requisition form “tdap™ EGFR” or “Comprehensive” and fax to TMD at 630.789.3765.
Ship patient specimens to TMD at 610 Oakmont Lane Westmont, IL 60559. If a paraffin block is not available, 10 unstained slides (cut in 4-5 micron sections) from each site may be submitted.

Specimen Requirements for all IHC including EGFR pharmDx assay
Paraffin-embedded tumor tissues preserved in the following fixatives are suitable for all IHC testing including EGFR pharmDx™: 10% (v/v) neutral buffered formalin; 10% (v/v) unbuffered formalin; 25% (v/v) unbuffered formalin; AFA (acetic formalin alcohol); Richard-Allen Scientific’s Pen-fix; and Bouin’s fixative. Tissues fixed in Anatech’s PreFer are not suitable for EGFR pharmDx™ or other IHC assays. PreFer fixation results in unsatisfactory preservation of morphology for IHC analysis and may cause erroneous results.

Specimen Requirements for FISH
Paraffin-embedded tumor tissues preserved in the following fixatives are suitable for FISH testing: 10% (v/v) buffered formalin; Paraformaldehyde; Karnovsky’s fix (16%paraformaldehyde, 50% glutaraldehyde); Trump’s fix. Tissues fixed in the following fixatives are not compatible with FISH: Buin’s fix; Prefer fix; B5 fixative; Non-buffered formalin; Any acid-based fixatives (picric acid, etc); Mercuric chloride; Hollande’s fix; Michelle’s fix; Zamboni’s fix; Zenker’s fix; Formaldehyde/zinc fixative; Alcohols (any when used alone). Tissue samples should be optimally fixed in buffered formalin for no longer than 40 hours.
